School Screenings

The Penfield School District's School Health Services program supports your student's academic success by promoting health in the school setting.  One way that we provide care for your student is by performing the health screenings as mandated by the State of New York. During this school year, the following screenings will be required or completed at school: 
Distance acuity for all newly entering students and students in Kindergarten, Grades 1, 2, 3, 5, 7 and 10.
Near vision acuity and color perception screening for all newly entering students.
Hearing screening for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7 and 10.
Scoliosis (spinal curvature) screening for all students in Grades 5 - 9.

Physicals and Sports Permission Forms

A current physical exam and permission from parents are required to play sports.

Before a student may try out for a sport, he/she must have a copy of a current physical exam on file in the Nurses’ Office and have the Athletic Program Permission Slip/Medical Recertification Form completed by his/her parent(s). Forms for physicals (Health Appraisal Forms) are available online or in the Nurses’ Office.


A physical exam with your own health care provider is preferred, as they generally are more familiar with your child.  A physical exam is good for 12 months from the last day in the month the physical was done. If the physical exam is good to start the season, the athlete may complete the season. In addition, students must have been immunized against tetanus within the past ten years.

The parent Sports Permission Form is available in the Nurses’ Office or on the Athletics website (see Forms and Documents) and must be completed, signed, and turned into the school nurse no earlier than 30 days before a sport is scheduled to start. Please note:  A separate permission form is required for each sport season that a student wants to participate in during the school year.
