Hello! My name is Dayna Fantigrossi and I am the Director of Health Services at The Charles Finney School. I have been a nurse for many years with experience in the hospital in the areas of medicine and surgery. I thoroughly enjoy working with the students here at Finney! Health Appraisals and Vaccines:

NYS Education Law requires an updated Physical Exam for school attendance for students in grades Pre-K, K, 1, 3, 5, 7, 9, and 11, and for ALL new entrants to the Charles Finney School.

  • Documents are required by NYS within 30 days of starting school.
  • Proof of required immunizations is required for all students in Pre-K through 12th grade to attend school.
  • Vaccines required for Pre-K through 12th and school attendance:
    • Hib (Pre-K only)
    • Pneumococcal (PCV) (Pre-K only)
    • Hepatitis B vaccine
    • Measles, Mumps and Rubella vaccine (MMR)
    • Polio vaccine
    • Varicella (Chickenpox) vaccine
    • Diphtheria and Tetanus toxoid-containing vaccine and Pertussis vaccine (DTaP or Tdap)
  • Additional vaccines required for middle and high school:
    • Tdap vaccine for Grade 6
    • Meningococcal conjugate vaccine (MenACWY) for Grade 7
  • Students in Grade 12 need an additional booster dose of MenACWY

Medication:

  • A permission form is required for your child to receive medication in school. Students are not permitted to carry any medication, unless there is permission from a doctor.
  • Medication must be dropped off to the health office by the parent and labeled with the child’s first and last name.
  • Permission to give any medication requires a form with your signature AND the physician’s signature. This applies to prescription and non-prescription medication and also for students who carry inhalers or Epi-Pens. Please click for Medication Authorization Form.

  • Both prescription and non-prescription medication must be in the original container. All medications must be delivered and picked up by a parent.

Health Concerns:

If your child has an allergy, asthma, diabetes, history of seizures, or a health related concern, please fill out a Care Plan with your child’s health care provider and parent signature, and drop this form off at the health office with any associated medication. School Sports:

When your child is eligible to participate in school sports, a sports form must be completed and returned to the health office for EVERY sport season. An updated physical is required within a year of the starting month of that season. Please click for Sports Form.
